Note: Replacement certificates and fee certificates will not be issued at night or on regular holidays; only certificates issued on the same day at the outpatient billing counter will be accepted.

Applicant preparation documents：  
1. In person: ID card (adult), household certificate (child), passport or residence permit (foreigner), entry/exit permit (Chinese national).
2. Legal representative (minor, person under guardianship declaration or auxiliary declaration). (1) The original of the patient's ID card (the minor may provide the original of the household registration book), (2) the original of the legal representative's ID card, (3) the proof of the relationship with the patient [household registration book, copy of household registration with records (six months) valid within), court rulings, etc.]. ※If you entrust others, you must prepare the above-mentioned materials, the entrustment agreement and the original ID card of the entrusted agent.
3.  For relatives of the deceased.
    a. Proof of the relationship between the immediate family member's identity card and the deceased.
    b. (1) The original identity card of the person with inheritance rights (spouse, children, or as stipulated in Article 1138 of the Civil Law).
        (2) Documents proving the relationship with the patient【The original copy of the identity card, recorded household certificate or household register (valid for six months), etc.】
        (3) The original certificate of removal of the patient from the household (① original transcript of removal of the household or ② original death certificate or ③ original certificate of autopsy).
   ※ If you appoint another person, you must have the aforementioned information, the consent letter and the original identity card of the appointed agent.
4. Principal to apply (1) the original identity card of the patient, (2) the original identity card of the proxy, and (3) the power of attorney issued by the patient.

If you are applying for a medical certificate that was issued seven years ago (the retention period for medical records is seven years according to the law), please call or write to us to confirm before applying at the hospital.

Birth Certificate
The first three copies will be priced at 100 NTD (including two free copies).
From the fourth copies onwards, the price will be 50 NTD per copies.
