COVID-19 self-pay test
To cooperate with the epidemic prevention policy of the Ministry of Health and Welfare, Mackay Memorial Hospital, by the Central Epidemic Command Center's "Regulations on Opening Some People to Test for COVID-19 (New Coronary Pneumonia Nucleic Acid Test) at their own expense", allows people who meet the requirements to go to Mackay for self-financed COVID-19 testing (New Coronary Pneumonia Nucleic Acid Test), and provide test certificates. Precautions
All documents must be prepared when reporting for inspection, and inspection can only be carried out after an on-site inspection.
1. Original ID card and passport.
2. Self-pay COVID-19 (New Coronary Pneumonia Nucleic Acid Test) application form (not attached if completed online).
3. Relevant documents for entry application. (according to the applicable regulations of the CDC)
4. E-ticket or proof of ticket purchase.
*In response to the severe international epidemic situation, many countries have required inspection certificates within 48 hours. It is recommended that you take the inspection two days before departure. If you want to go earlier, you must confirm with your airline whether it is acceptable.
*For people who go out of the country for inspection at their own expense, each person should apply twice within 6 months as a principle and must cooperate with home quarantine and other related epidemic prevention measures when returning to Taiwan from abroad.
*This service only provides self-pay testing operations. If you have other medical needs, please go to the outpatient clinic or emergency department for medical treatment.
*Standards related to missed appointments:
1. If it is the first time a patient comes to Mackay Hospital for a self-pay examination, the outpatient medical record number will be automatically generated, which is regarded as a first visit.
2. If you are unable to make an appointment, please cancel the appointment in the "Enquiry and Cancellation" function, so as not to miss the appointment and affect your rights for future online registration.
