Notes on Registration

◎   For the convenience of the public, we can make more use of day/evening outpatient clinics .
◎   Peace to all citizens. Recently, there are some online businesses that provide registration services and charge fees, which is not a formal business procedure of the hospital. We remind you to protect your personal information carefully to avoid affecting your rights and causing property damage. If you need to make an appointment, you can do so through the Internet, our mobile registration app, the clinic counter, operator voice service, or a self-service payment machine. MacKay Memorial Hospital cares about you!
◎   We accept appointments within 4 weeks. For chronic prescription holders, they can pre-register at the counter, by phone or at a self-service kiosk for an appointment one month after their last prescription.
◎   If you are under 18 years old, please register for the clinic. To prevent inoculation, please register at well baby clinic in the afternoon.
◎   First-time patients can (a) use the voice call, APP registration, and then come to the hospital to fill out the initial consultation form；(b) Register online and fill out the initial consultation form online. Before the registration time closes at noon on the day of the visit, go to the registration counter with the initial consultation form (voice call, APP registration), health insurance card, and ID card (or household certificate) to register for the initial consultation. (Check-in time: before 11:30 a.m., before 16:00 p.m., before 20:00 p.m. for night clinic)

◎   If there is any change in the patient's basic information, please fill out the "Application for Change of Patient's Personal Information Form" and bring both documents such as health insurance card and ID card (for minors: household certificate or household registration transcript) to the registered billing counter to apply for the change of information in a timely manner. If you are unable to attend the clinic on the day of your visit, please cancel your registration half an hour before your doctor's appointment (8:30 a.m., 13:30 p.m., 17:30 p.m.) by using the Internet, the Mackay Mobile Registration App, or the voice call. If you fail to cancel your registration, you will be considered to have missed your appointment. If you have missed your appointment three times, you will not be able to register online or by phone. Please use on-site registration to cancel the number of missed appointments.
◎   Please be sure to check out at the billing desk after your visit, regardless of whether you have a prescription or not. If the bill is not settled on the day of the visit, it will be listed as an outstanding fee from three days onwards and must be paid before another appointment can be made. We do not accept telephone appointments for self-appointment by doctors.
◎   The teaching clinic may take longer for medical history and teaching discussion. If you do not agree with the presence of the student interns, please feel free to ask and we will cooperate.
Our hospitals (Taipei Mackay Memorial Hospital, Mackay Children's Hospital, and Tamsui Mackay Memorial Hospital) can accept registration and price approval from each other.
◎   Morning clinic, afternoon clinic, and evening clinic are each open for 2 people in same-day registrations, please use the Internet / Mackay Mobile Registration App / Voice Call Registration:

     (3) Example description： Wang Xiaoaming's medical record number wanted to be registered with Dr. Lin Hyunpui in the morning of August 15, but he forgot his medical record number and only remembered his ID card number. A123456789

Step 1: Press 1 for Mandarin, 2 for Taiwanese, 3 for English, and 2 for Taiwanese
Step 2：Press 1 for appointment registration, press 2 for cancellation registration, press 3 for inquiry registration, press 4 for operation instruction, press 1.
Step 3: Press 1 for medical record number, press 2 for ID number, press # after entering. Press 2, then enter 01123456789 #
Step 4: Enter the appointment date code (month and day) with four codes. Suppose it is August 15, press 0 8 1 5
Step 5: Enter the clinic hours: 1 in the morning, 2 in the afternoon, 3 in the evening, and 1 in the evening.
Step 6: Enter the doctor's code in four codes, if you do not specify a doctor, please press #. Press 4535 (Dr. Lin Hyun Pui)
Step 7: If you do not designate a doctor, enter the department code for a total of two codes. Press 30 (Pediatrics)
Step 8：The voice system will tell you the date and consultation number, the time you should be seen, etc.
