How do I apply for a "General Diagnosis" or "Special Diagnosis"? |
1. Outpatients: Apply only after registering and being diagnosed by a doctor. 2. Inpatients: Apply to the ward nursing station before discharge. If you do not apply at the time of discharge, you will have to register again for outpatient service and get a certificate from the attending physician of the original department. 3. Applicants must present their ID card (for adults), household certificate (for children) or passport (for foreigners) for verification of identity. 4. The delegate should present the original identity card of the delegate, the original identity card of the delegate, and the letter of delegation for application. |

How do emergency patients apply for a diagnosis? |

How do I apply for medical records such as "Medical Record Summary" and "Photocopy of Examination Report"? |
1. To protect patient rights and privacy, please provide the following documents with your application. ◎ The patient himself/herself: the original of his/her identity card (for non-nationals, please provide the original of passport or residence permit). ◎ Proxy: (1) original patient ID card, (2) consent letter, and (3) original ID card of the proxy to be appointed. ◎ Legal representative (minor, person declared under guardianship or auxiliary declaration): (1) original identity card of the patient (if minor, original household certificate), (2) original identity card of the legal representative, (3) proof of relationship with the patient (original and photocopy of household certificate, household register, court ruling, etc.). If the patient appoints another person, the above information, the consent letter and the original identity card of the appointee must be provided. ◎ For those who have the right of succession (information for those who apply for death): (1) the original identity card of the person who has the right of succession, (2) proof of relationship with the patient (original identity card, household certificate, recorded account transcript, etc.), and (3) original proof of removal of the patient from the household (①original transcript of removal of the household or ②original death certificate and recorded household certificate)。※ If the patient is entrusted to another person, the above information, the consent letter and the original identity card of the entrusted agent must be provided. 2. How do I get a replacement or additional certificate of diagnosis? |
The applicant or his/her family members should present the original ID card (the trustee should present the original ID card of the applicant, the original ID card of the trustee, and the commission letter) to the certificate counter and apply for $20 each. |

How do I get a replacement receipt? (The difference between a lost replacement and an additional copy) |
1. The original medical receipts cannot be replaced if lost, but you can apply for a fee certificate at the certificate counter for $50 per copy. 2. For additional copies, please visit the Certificate Counter on the 1st floor of the Inpatient Center from 08:30 to 16:30, Monday to Friday, to apply for duplicate receipts at NT$20 per copy. |

How do I apply for a "Chinese Birth Certificate" or "English Birth Certificate"? |
1. 1.Chinese birth certificate ◎First time application: The parents of the newborn baby apply to the ward nursing station with the original national identity card. ◎ Application for reissue: Apply at the certificate counter. ◎ Acceptance time: Monday to Friday 08:00 to 16:30 2. English Birth Certificate ◎ Acceptance time: Monday to Friday 08:00 to 15:00 ◎ Application Method: (1) Inpatients should apply at the ward nursing station. (2) If you apply for the English version (you can apply by e-mail in advance, and then notify us to pick up the application) or apply for the Chinese version before, you can only apply directly to the inpatient center by your immediate family members, and you must present your documents. ◎ Application fee: NT$200 for the first copy and NT$20 for the additional copy. ◎ Where to apply: Certificate counter.
