I. How to apply
• ‘’Live Counter Application’’, "same-day clinic application", "e-mail application";For inpatients, please contact the ward nursing station.※The "E-mail application" method is described in the second point.
• If you are applying for a discharge summary, please wait at least 5-7 days after discharge from the hospital before applying.
• Waiting time at the counter for medical records is about 40 to 60 minutes;If you have just been discharged from the hospital, have not been seen for more than 2 years, or have died, or have a summary of your medical record in English or Chinese, you will be notified by phone of the time you can pick up your medical records.
• When applying for/receiving the documents, the original and photocopies of the identity documents should be examined and retained.
• If you do not receive your application within 1 month of the notification date, it will be considered invalid.
• Service hours: Monday to Friday (as stated in point 6). ※ Please apply by e-mail on Saturdays, Sundays and nationalholidays, after receiving the mail during the service hours (Monday to Friday), the contractor will take the initiative to confirm by phone. II. E-mail application
• Download and complete the "Request for Photocopy of Medical Records (including Consent Form)" and send it as an electronic file attachment to mmhmrdcopy@mmh.org.tw. After receiving the mail during the service hours (Monday to Friday),the contractor will take the initiative to confirm the mail by phone and notify the on-site counter within 3-7 days to collect the documents. The documents that should be prepared are described in point 4.
• E-mail open application time: 24 hours;After receiving the mail during the service hours (Monday to Friday), the contractor will take the initiative to confirm by phone.
• Have all documents available at the time of application (as stated in point 4);If you appoint someone else, you must have the original consent form signed by both parties.
• You need to pick up the documents at the counter (due to the need to check the identity documents, we do not provide mail); The location and time of pickup is as stated in point 6.
• Payment method: Payment is made at the time of collection at the counter.
III、Application Process
• Fill out the application form (you can apply at the counter or bye-mail).
• Numbered cards will be drawn on the spot.
• The original and a photocopy of the identity document (as described in point 4) are required; if you entrust someone else, a signed consent letter from both parties is required.
• Follow the light to apply.
• Waiting for pickup (if you can't pickup on the same day, we willcall you to inform you of the pickup time).
• Payment (as described in point 5).
IV. Required Documents (Application and Documents Must Be Presented for Verification)
• The patient himself/herself: the original of his/her identity card (for non-nationals, please provide the original of passport or residence permit).
• Proxy: (1) original patient ID card, (2) consent letter, and (3) original ID card of the proxy to be appointed.
• Legal representative (minor, person under guardianship declaration or auxiliary declaration). (1) the original identity card of the patient (for minors, household certificate), (2) the original identity card of the legal representative, (3) proof of relationship with the patient【The original copy of household certificate, recorded account transcript (valid for six months), court ruling, etc.】※If you appoint another person, you must provide the aforementioned information, the consent form and the original identity card of the appointed agent.
• For those who have the right of succession (information for those who apply for death). (1) The original identity card of the person with inheritance rights (spouse, children, or as stipulated in Article 1138 of the Civil Law), (2) Proof of relationship with the patient【 The original copy of the identity card, recorded account book or household register (valid for six months), etc.】(3) The original certificate of the patient's household certificate (①The original copy of the household certificate or ② the original death certificate or ③ the original certificate of autopsy)。 ※ If you appoint another person, you must have theaforementioned information, the consent letter and the original identity card of the appointed agent.
V. Charges (Denominated in New Taiwan Dollars, NTD)
• Medical records fee: Basic administrative fee of 200 NTD per visit (within 10 pages), additional 5 NTD per page over 10 pages.
• The fee for the first optical disc of medical images is NT$200 for each examination, NT$400 for the second, and NT$500 for the third (inclusive) or more, and 20% of the first fee is added for each additional disc.
• Chinese/English medical record summary fee (need to ask the doctor to reorganize): 400NTD/section.
• We provide mailing service, if you need, please contact the counter to fill out the envelope and pay the mailing operation fee of NT$100 (medical records are charged separately);※ Since CD-ROMs are prone to breakage, only paper copies can be sent by mail (E-mail applicants are not provided with mail due to the need to check identity documents).
VI. Application Locations and Service Hours Are As Follows.
