1. Please submit the referral form to the doctor for reference during the visit
2. The referral is limited to one outpatient service
3. Part of the cost of $170, not covered by health insurance

   Charges are based on the Hospital's fee schedule
4. If the doctor determines that the referral is necessary for continued outpatient treatment, the fee for four visits within one month from the date of referral will be the same as the referral fee

Emergency

Go to the emergency examination and registration counter for registration

3. The first and second level patients of the injury classification will pay $450, and the third, fourth and fifth level patients will pay $550

Hospitalization
