1. Please apply directly to the doctor at the time of the emergency visit and receive it on the same day.

How do I apply for a diagnosis of death in English or Chinese?

1. If the death certificate can be issued by the physician on the same day, present the patient's ID card and the ID card of the immediate family members to the emergency fee counter at the time of checkout.

2. If you need to apply for an English death certificate on a day other than the same day, you need to make a separate outpatient appointment and have it issued by a physician.

3. If you have already applied for a copy of the Chinese (English) death certificate, please visit the certificate counter at the Inpatient Center on the 1st floor from Monday to Friday from 08:30 to 16:30.
